Supplemental Health
All FPOA members are entitled
to receive supplemental benefits for the medical, vision and prescription
bills they incur. Medical benefits pick up deductibles and co-payments as
established by the Fresno City Employees Health and Welfare
Trust. Benefits are also available for vision and hearing aid expenses, as
well as chiropractor, podiatrist, and counseling services. Revised benefits
are offered to members aged 65 and over. For details, call Jo

Does the FPOA pay my $200 per fiscal
year medical deductible? |

A. |
Yes, as long as the deductible is taken
from an allowable expense. |

Do I have 100% health coverage at all
times? |

A. |
In most cases, the FPOA, by coordinating
benefits with the Trust, provides 100% coverage for your medical
expenses. Exceptions include dental, and any expense deemed
ineligible by the FPOA and/or the Trust. |

When I retire, do I have to maintain the
City’s health plan in order to be eligible for the FPOA benefits? |

A. |
No. You do not have to keep the City’s
plan; however the FPOA will coordinate benefits with whatever plan
you do have and will pay you no more than it would have if you had
retained the City’s plan. |

Is my family entitled to the

What if my Physician is not on the list
of Blue Shield Providers? |

You can go to any doctor you choose.
But know this: If the doctor you choose is not contracted with
Blue Shield, you will be responsible for a 50% co-payment and you will have to pay
for any expenses that exceed the trust’s scheduled allowable. |

What should I do before I see a Blue
Shield contracting physician? |

You do not
have to do anything. However, if the visit requires
pre-certification,
you must call Blue Shield at 800-343-1691. See
your current Fresno City Employees Health & Welfare plan booklet
for a list of services that require pre-certification or contact
Jo Billings at the FPOA office (559-442-3762) for this
information. |

How do I receive FPOA supplemental

For Medical Expenses |
| |
Have your
provider bill your primary carrier (which, for all active members is
Health Comp) and your secondary carrier, the FPOA. If your provider
will not bill the FPOA, submit a copy of your EOB from your primary
carrier along with the corresponding itemized bill to the FPOA. Please
let us know if you want us to pay benefits to you or the provider.
Also, be certain to tell us if your illness/injury is work-related. You
can use a FPOA claim form to convey this information to us, or you can
write the information on the back of the bill or EOB. |

For Prescription Expenses |
| |
Use your City’s Health & Welfare identification card (Health Comp) to
purchase your prescription drugs. (Retired members who do not have the
City’s plan should use whatever drug benefit program available to
them.) Unlike medical expenses, no EOB’s are required for prescription
expenses. Simply send the drug tag (not the cash register receipt) from
the pharmacy to the FPOA office within 18 months from the dispensing
date for reimbursement. |

Life Insurance |
|
The FPOA offers an assortment of term life insurance polices, all at
very competitive group rates. Active FPOA members receive a $10,000
life insurance policy, $20,000 if death is accidental. (The policy
reduces in value after age 65.) Active members also receive an
accidental death and dismemberment policy that pays a minimum of $1,000
if death was off-duty, or $5,000 for an on-duty death. Most members
elect to purchase additional life insurance. For further

Long Term Disability Insurance |
|
This optional policy is an extraordinary value that pays 2/3 of the
salary you were making at the time you became disabled after a 30-day
waiting period (0 days for industrial disabilities.) For
non-industrial disabilities, after 12 months of being eligible for
disability, the benefit is adjusted for cost of living, up to 5% per
year. For most disabilities, this policy pays benefits for at least 2
years. (One year for mental disabilities.) After two years, if you are
totally disabled, benefits may continue until death. All payments are
coordinated with any payments you may get from the City. . .which means,
if you are getting at least 2/3 of your salary from the City, you will
not receive additional payments from this policy. Maximum benefits are
realized on non-industrial disabilities, and in some cases where the
officer is forced to retire due to a disability. Because you pay for
this policy with after-tax dollars, LTD benefits are not taxable. The
policy also provides a $50,000 death benefit and additional survivor=s
benefits. The monthly premium for this policy is $19.50. For further

Salary Continuance |
| This
benefit pays 2/3 of existing base salary for the first 30 calendar days
of disability for any active FPOA member who is unable to work due to a
disability and is receiving no income. (Certain restrictions apply.)

Scholarships |
| Since the
inception of this benefit in 1994, over $75,000 has been given to
dependent children of FPOA members to help offset the high cost of
college. Applications are usually available in February and
scholarships awarded in May. An eligible applicant must (1 ) be an
unmarried dependent child of an active or retired member of the FPOA in
good standing, or the unmarried child of a deceased member of the FPOA,
(2) have achieved a minimum grade point average of 2.5 (based on the 4.0
system), as evidenced by transcripts, and (3) be enrolling in the
following Fall semester for either full-time (12 or more units) of
undergraduate college study, or in an accredited vocational school.
(Scholarships are not awarded to applicants who have already received
four FPOA scholarships, or for postgraduate studies.) |
